Care Community Services provides person-centered developmental disability support services designed to help individuals with disabilities and those requiring medical support transition into safe living environments. We were established with a clear purpose: to promote independence, stability, and meaningful community connection through supportive housing and coordinated care.
We believe that living in a home setting helps individuals build stronger community ties while maintaining dignity and independence. Care Community Services offers hands-on support throughout the entire transition process and remains actively involved after relocation to ensure each individual is adjusting well, receiving the right supports, and thriving in their home environment.
